Our Comprehensive Medical Day Care Program is uniquely designed in a 16,000 square foot facility and is fully funded by Medicaid, we also accept other funding sources along with a private pay rate. We strive to create a program that provides care for our client. We strive to develop clients into an essential component in the community. Our facility includes but is not limited to the following: Nursing and Medical Services: Registered Nurse(s) on premises at all times for health monitoring, care planning, medication distribution and care management. Transitional Care coordinated with the Primary Care Physician, the client and the caregiver. Our entire Medical component is overseen by our Medical Director.
